False PHD is a passive item introduced in The Binding of Isaac: Repentance. It is an evil-themed counterpart to the regular PHD item. When picked up, it gives Isaac one Black Heart and alters how pills behave. Core Effects
Pill Identification: It identifies all pills instantly. You no longer have to eat a pill blindly to see its name.
Pill Corruption: It converts many “good” stat-modifying pills into their “bad” counterparts.
The Big Benefit: Every time you consume a stat-decreasing pill, it grants a permanent Damage Up (+0.60) modifier.
Black Heart Spawns: Consuming non-stat bad pills (like Amnesia, Addicted, or Paralysis) drops a Black Heart on the floor instead of boosting your damage. Key Item Synergies
PHD / Lucky Foot / Virgo: If you hold these items alongside False PHD, pills can be both good and bad again. However, whenever you do take a bad pill, you still trigger the Damage Up or Black Heart rewards from False PHD.
Placebo: Allows you to repeatedly copy a bad pill’s effect to farm infinite Black Hearts or stack massive damage multipliers.
A Pony / White Pony: These items lock your movement speed to a minimum baseline of 1.5. This lets you consume Speed Down pills for free damage without losing actual movement speed.
